Current Open Positions:
Home Health Provider Contractor (compensation based on needs of the client) Empowered Care is seeking compassionate individuals or families to become Host Home Providers and open their homes to support adults with developmental disabilities. As a Host Home Provider, you will offer a safe, stable, and nurturing environment where individuals can thrive while promoting independence and community integration.

Key Responsibilities

Provide a welcoming and supportive home environment.

Assist with daily living activities, including personal care, meal preparation, and transportation.

Encourage and support community involvement, social engagement, and recreational activities.

Help individuals develop life skills and achieve personal goals.

Administer medications and ensure medical appointments are attended as needed.

Maintain accurate records and communicate regularly with Empowered Care staff.

Day Program Mentor ( $17.00 to start) In this role, you will collaborate with participants to facilitate program-approved activities that foster community inclusion and encourage personal growth. Our mission is to help individuals with disabilities become integral members of society, promoting their autonomy and decision-making in choosing activities that enrich their lives.

Key Responsibilities

Engage with individuals in community activities, supporting them in achieving personal goals and aspirations.-

Collaborate closely with clients, providing guidance and encouragement.

Implement the approved calendar of community activities, ensuring a diverse range of options.

Supervise and be responsible for 1 to 4 individuals during program sessions, maintaining a safe and supportive environment.

Advocate for the needs and preferences of each client, ensuring their voices are heard.

Maintain accurate records and documentation, preparing necessary reports as required.

Foster positive interactions and healthy relationships within the community.

Work collaboratively as part of a dedicated team.

Personal Care Provider ( $17.00 to start) As a Personal Care Provider, staff members will work with individuals that have a diagnosis of Intellectual Developmental Disabilities (IDD) in their home as well as in the community. While in the home, the Personal Care Provider (PC) will help to promote safe and healthy daily living activities by working side-by-side with the client

Supported Employment Job Coach ( $17.50 to start) As a Supported Employment Job Coach Specialist, you will play a vital role in assisting clients with developmental disabilities as they pursue and maintain employment. You'll work closely with clients to assess their strengths, create individualized employment plans, and offer hands-on guidance in job tasks and social interactions. The ideal candidate is compassionate, patient, and skilled in empowering others to achieve their personal and professional goals.

Key Responsibilities

Develop personalized employment plans for clients based on their unique strengths, goals, and ambitions.

Work side-by-side with clients on-site to teach job tasks and help them develop necessary skills.

Communicate with clients regularly about their career goals, aspirations, and personal barriers, providing support to help them overcome challenges.

Assess clients' strengths and guide them in using these effectively within the workplace.

Assist with social skill-building and life skills to ensure successful workplace integration.

Advise clients and employers on appropriate workplace accommodations to support clients' needs.

Keep accurate records of client progress and prepare detailed reports to document achievements and areas of development​

Associate degree or higher (or currently pursuing a degree).

Proficient in Microsoft Office, with skills in creating and editing documents, including resumes.

Strong communication skills, with the ability to read and write proficiently.
